To Our Metlife
Patients: As of June 1st, 2008, our office will no longer be a Metlife PPO provider, we will become an out-of-network provider. We want you to know that we are glad to help you through this transition and we want to retain you as a patient while continuing to provide you with the same excellent dental care for you and your family. Our office will be happy to continue to file your dental claims with Metlife. Preventative dental care will still be covered by your insurance company with no out-of-pocket cost required from you. Your patient portion for basic and major dental procedures will remain the same as we collect from your in-network fee schedule. |
