Important information about MetLife
Beginning June 1, 2008, Dr. Freedman & Associates will no longer be participating with the MetLife Preferred Dentist Program. This means that our office will be an Out of Network provider, all percentage or patient portion are due at time of services. In order to have your claim submitted to MetLife via our office, we will require that you bring a copy of your insurance card and benefits at the time of your visit. If you are not able to bring your insurance information, you will be required to pay your balance in full and we will provide the appropriate paperwork for your claim submission.

We will gladly submit to your primary insurance company for all treatment rendered and accept direct reimbursement from the insurance company. Co-payments for amounts not covered by your insurance will be due when services are rendered. Any pretreatment estimates will be submitted to provide you with the most accurate information regarding your coverage and co-payments. All insurance information is entered into our computer system and certain claims are sent electronically to expedite processing payment. For your convenience, MasterCard, Visa, personal checks, and cash are also accepted.

Our Goal is to make your dental experience comfortable and pleasant. For your convenience, our office accepts numerous indemnity insurance plans as well as many Managed Care and PPO plans.
